Before Your Appointment
- If you are a new patient, please arrive 10-15 minutes before your appointment
- Please bring your:
- Medicare Card and/or any Health Care Card, Pension Card or Health Insurance Card.
- Details of your current and previous medical conditions
- Details of any medications you are currently taking
- Referral letters
- You are welcome to bring a friend or relative
- Children under 12 must be accompanied by a parent or guardian
- It is preferable that mature minors (ages 13 – 16) be accompanied by a parent or guardian, but is not required.

Bulk Billing
Please Note:
From Monday 2nd September 2019, we will NO longer be a routinely Bulk Billing Clinic and our Practice Fees will differ slightly.
Please see detailed Fee structure below for more information.
Billing is at the discretion of the GP.
Some exceptions may apply.
No Bulk Billing on Saturdays.

Cancellation Policy
Please inform us at least 24 hours prior to your appointment if you need to cancel or reschedule your appointment, to avoid a cancellation fee. This allows us to offer the appointment slot to other patients also in need.

Patient Registration Form
New patients who have not been seen at this practice before can print this form, once completed it should be brought along to you initial appointment
Records Transfer Request Form
The records transfer request form is for those patients wanting to have their past medical records sent through to our surgery. This can be helpful for you doctor to look over past medical history and medications to help with any current conditions. Once completed it can be given to our receptionists who will fax it through to your previous doctor.
Strathmore Family Medical Centre Billing Policy
Strathmore Family Medical Centre still strives to maintain its commitment to providing high quality medical care to the local community. Any individuals who may suffer financial hardship and are unable to pay our standard rate are encouraged to still book an appointment, and discuss the financial implications with your GP at the time of consult.
